The purpose of this study was to describe the extent to which nurses perceive the ethical dimensions of clinical practice situations involving patients, families and health care professionals. Using the composite theory of basic moral principles and the professional standard of care established by legal custom as a framework, situations involving ethical dilemmas were gleaned from the nursing literature. They were reviewed for content validity, clarity and representativeness in a two-stage process by expert panels. The situations were presented in a (...) written format to a convenience sample of nurses (n = 125), who were primarily staff nurses (65.6%). Respondents' judgements about whether the main issue of each situation concerned ethics ranged from a low of 0.8% to a high of 40%. From analyses of the categories into which the majority of subjects placed each situation, it was concluded that these nurses generally perceived ethics as the main issue in situations that directly involve patients' autonomy. Analysis yielded unanticipated findings about the themes in ethical situations to which nurses in practice may respond. (shrink)

Certain characteristics of childhood immaturity (e.g., infantile facial features) may have been favored by natural selection to evoke positive feelings in adults. We propose that some aspects of cognitive immaturity might also endear young children to adults. In two studies, adults rated expressions of mature and immature thinking attributed to children. Immature thinking in which children expressed a supernatural explanation elicited positive affect reactions, whereas other forms of immature thinking, which made no attribution to supernatural causation, were responded to negatively. (...) This pattern was found for parents and others, males and females, American and Spanish college students, and for target children 3 to 9 years of age. We suggest that persistence of supernatural thinking in adulthood causes people to view the expressions of such thinking in children in a positive manner, fostering nurturance of young children who display them. This article analyses the accusation of nihilism that F. H. Jacobi made against Fichte in a letter of mars. 1799, imputation that lays on the personal conception ofJacobi about the reason as a negative capacity which only can destroy its objects. The letter in question implicates two different types of nihilism: a)The cosmological one, accepted by Fichte and named acosmism by him. It is only a reformulation of the sensible world from practical principIes, but doesn't suppose an annihilation or scorn (...) of this world. Qn the contrary, the religious view, for example, shows the world as a manifestation of godlite and, hased on moral exigencies, doesn~t admit the existence of death. Of course, the immortality isn't for a particular soul but for the order of moral actions (= God). b) The radical one, which doesn't exist in Fichte and lays on a bad interpretation of bis intellectual intuition assumed from a point of view next to Schelling. This essay raises the question of gender in relation to the question of intellectuals in Britain, commenting on the gender blindness that made their exclusion so automatic in Collini's study. It looks at some women who might have been included, focussing particularly on Virginia Woolf as one who was not only a very significant public intellectual, but who in her essays entitled 'The Common Reader' also provided a definition and analysis of the role of an intellectual which is very (...) different from that suggested by Collini. This lavishly illustrated book establishes the towering influence of the scientist Victor Regnault in the earliest decades of photography, a period of experimentation ripe with artistic, commercial, and scientific possibility. Regnault has a double significance to the early history of photography, as the first leader of the Société Française de Photographie and as the maker of more than two hundred calotype portraits and landscapes. His photographic and scientific careers intersected a third field with his appointment in 1852 as director of (...) the Sèvres porcelain works. Readers are treated to Regnault's own beguiling pastoral, garden, and forest scenes; striking portraits of the scientists and artists in his circle of friends; quirky images of acoustic experiments; and an insider's view of the Sèvres porcelain works. Regnault's richly varied photographs also encompass perhaps the most extensive group of family portraits in early photography, and his romanticized landscapes reflect a moment when the rural outskirts of Paris were being aggressively suburbanized and industrialized. Occupying a unique and powerful position in the overlapping spheres of photography, science, industry, and art, Regnault was elected president of the newly formed S.F.P. in 1855. Kentucky and West Virginia are among the states most severely affected by opioid poisonings and deaths. The legislatures of both states have enacted a broad range of bills intended to address related issues. We present an overview of legislation enacted in 2017 and 2018, along with an approach to analysis of practitioner response to one type of legislation.

Nicholas Wolterstorff's Divine Discourse attempts to give philosophical warrant to the claim that ‘God speaks’. While Wolterstorff's argument depends largely on his appropriation of J.L. Austin's speech act theory, he also uses two narratives that for him demonstrate how ‘God speaks’. The first is the story of Augustine's conversion in the Confessions and the second is a story that Wolterstorff recounts about a certain ‘Virginia’. This study argues that what Wolterstorff claims to derive from Augustine's narrative for his view (...) of divine discourse is not fully supported by the Confessions, and that Augustine's interpretation of the words ‘tolle lege, tolle lege’, can be construed as a useful interpretation of an ambiguous sign. This is consistent with Augustine's understanding of the interpretation of texts in both the De doctrina christiana and the Confessions. In short, Augustine is far more open to the ambiguity of signs than Wolterstorffs's account suggests. (shrink)

ObjectiveTo compare techniques for computing clustering and switching scores in terms of agreement, correlation, and empirical value as predictors of incident cognitive impairment.MethodsWe transcribed animal and letter F fluency recordings on 640 cases of ICI and matched controls from a national epidemiological study, amending each transcription with word timings. We then calculated clustering and switching scores, as well as scores indexing speed of responses, using techniques described in the literature. We evaluated agreement among the techniques with Cohen’s κ and calculated (...) correlations among the scores. After fitting a base model with raw scores, repetitions, and intrusions, we fit a series of Bayesian logistic regression models adding either clustering and switching scores or speed scores, comparing the models in terms of several metrics. We partitioned the ICI cases into acute and progressive cases and repeated the regression analysis for each group.ResultsFor animal fluency, we found that models with speed scores derived using the slope difference algorithm achieved the best values of the Watanabe–Akaike Information Criterion, but with good net reclassification improvement only for the progressive group. For letter fluency, different models excelled for prediction of acute and progressive cases. For acute cases, NRI was best for speed scores derived from a network model, while for progressive cases, the best model used clustering and switching scores derived from the same network model. Combining variables from the best animal and letter F models led to marginal improvements in model fit and NRI only for the all-cases and acute-cases analyses.ConclusionSpeed scores improve a base model for predicting progressive cognitive impairment from animal fluency. Letter fluency scores may provide complementary information. (shrink)

Childress (bio)A quarter of a century ago, in my second year of teaching at the University of Virginia, I began to explore the emerging field of biomedical ethics through a seminar on “Artificial and Transplanted Organs,” which included both faculty and students from law, medicine, and the humanities. My paper for the seminar was entitled “Who Shall Live When (...) Not All Can Live?” This experience drew me into biomedical ethics, because of the complex mix of ethical, legal, and medical issues and the fruitfulness of interdisciplinary and interprofessional dialogue.Several of the major problems confronting that seminar have persisted, often in altered form. In the area of organ transplantation, two significant issues continue to require society’s attention: (1) how can the organ supply be increased, and (2) how should the available organs be allocated? I want to make a few points about ethical criteria in the allocation of donated organs, as a way to indicate some directions for organ allocation policies in the twenty-first century. I will focus on the second question because I believe that a tremendous and ever-widening gap will persist between the need for transplantable organs and the supply of human cadaveric organs.Community Ownership of Donated Organs and Public ParticipationSince the mid-1980s, the operative conception of the ownership of donated organs has shifted, with major implications for the procedures used to formulate policies for organ allocation. As vice-chair of the federal Task Force on Organ Transplantation in the mid-1980s, I did not immediately realize that debates about allocation policies often reflected different conceptions of the ownership of donated organs. More specifically, organ allocation policies largely presupposed that donated organs belonged to, or were under the dispositional authority of, transplant surgeons, with only limited public accountability. However, the Task Force (1986) held that donated organs belong to the community, and that transplant professionals are only trustees and stewards of those organs for the [End Page 397] community’s welfare. This conception of ownership implies that the public should participate in setting the criteria for organ allocation.According to Jeffrey Prottas (1994), another member of the Task Force, these shifts represent the “socialization” of organ transplantation. Although professional dominance, through knowledge and power, continues in the United Network for Organ Sharing (UNOS)—the national organ procurement and transplantation network that was established in the late 1980s—that dominance is now more circumscribed and accountable, particularly because UNOS, in developing any allocation policy, has to respond “to public criticisms with public answers.” Not only is organ allocation policy now in the public domain—and thus a matter of public ethics rather than medical ethics—the terms of the debate have changed, as Prottas further notes, so that equity must be considered along with efficiency.Balancing Several Ethical Principles or ValuesThe main ethical criteria for organ allocation express principles that are prominent in the major competing theories of justice, particularly libertarian, egalitarian, utilitarian, and communitarian theories. The principles of liberty, equality, utility, and community all play important roles, and organ allocation policies often seek to balance these competing principles, which frequently are specified or circumscribed—for instance, utility is specified as medical utility. Just or equitable policies are found in the process of balancing.UNOS (1994) attempts to develop an equitable organ allocation system in light of a “set of basic principles” and “specific measurable objectives” that have been derived from these principles. It uses the metaphor of “balance” for its efforts to relate these different principles and objectives. For instance, it holds that balancing requires that “equal consideration” be given to both medical utility and to justice, so that neither can be ranked a priori over the other. However, it may not be possible to “give equal weight to [both] medical utility and justice” (UNOS 1994, p. 123) because trade-offs appear to be unavoidable in many policies and are consistent with the metaphor of balancing.Medical Utility and EqualityMedical utility—maximizing the welfare of patients suffering from end-stage organ failure—includes, at a minimum, attention to the factors that influence both graft and patient survival, as well... In 1924, Virginia Woolf wrote a short story based upon the life of Eleanor Ormerod. A wealthy spinster, Ormerod achieved notoriety in late nineteenth-century Britain as an economic entomologist. In 1904, Nature compared her to Caroline Herschel and Mary Somerville. In terms of recent scholarship devoted to the history of women in science, Ormerod's career differed markedly from that of her two predecessors. The emotional or intellectual support of a brother, husband, father, or male family relation made no considerable (...) contribution to her commitment to the study of entomology. Furthermore, her life as an independent spinster offered no positive proof for Francis Power Cobbe's dictum: as she aged, Eleanor Ormerod showed no tendency to become a ‘women's rights woman’. She publicly accepted or internalized the dominant, masculine ideology of science; and by contemporary standards, she achieved success. (shrink)

The issue of public health ethics has received much attention in recent years and is seen as a new field, distinct from medical ethics. Faculty from the University of Virginia, Johns Hopkins School of Public Health, Georgetown University, the University of Minnesota, and others received a grant from the Greenwall Foundation to examine this new field of public health ethics and identify the unique principles that distinguish it from the study of medical ethics. In the course of that study, (...) which included exploring the field with public health practitioners, a number of distinguishing ethical principles emerged. The moral principles appropriate for public health officials included producing benefits; avoiding, preventing and removing harms; producing a maximum balance of benefits over harms; and distributing benefits and burdens fairly. Beauchamp, editor. The Clarendon Edition of the Works of David Hume. New York: Oxford University Press, 1998.An edition of Hume's philosophic writings on rigorous, modern bibliographic principles has long been a scholarly desideratum. Readers in the many fields in which Hume's thought and style have made a profound impression have (...) basically relied on the originals or derivatives of efforts of the Victorian era. One is Hume's Philosophical Works (4 v., London: Longmans, Green & Co., 1874-75), edited by the Oxford dons, T. H. Green and T. H. Grose. Well-known for his idealist philosophy and theory of liberalism, Green contributed introductions to the first and second volumes criticizing Hume from an anti-empirical standpoint. Grose offered a "History of [Hume's] Editions" as an introduction to the third volume, and claimed to have revised Hume's text throughout. This edition has the virtue of comprehensiveness, but is deficient in its offering of variant readings. Later there came from Oxford University Press editions of the Treatise of Human Nature (1888), deemed generally sound, and the two Enquiries (1894), marred by departures from Hume's text, prepared by a prominent civil servant, Sir Lawrence Amherst Selby-Bigge, whose chief contribution was to provide useful analytical indices. An attempt at achieving higher editorial standards was made by P. H. Nidditch, Professor of Philosophy in the University of Sheffield, in a "second" edition of Selby-Bigge's Treatise (1976) and a "third" edition of his text of the Enquiries (1975). Limits were imposed by Oxford University Press, apparently unwilling to make more then small changes in the existing pages. Nidditch announced (Enquiries, 1975, at p.348, n.2) that he had a "fresh" edition in preparation, but he did not live to complete this project.In the event, the challenge to produce a critical edition of Hume's philosophical, political, and literary writings was canvassed at early meetings of the Hume Society formed in 1973. Thereafter, in 1982, a team of general editors was formed by T. L. Beauchamp, D. F. Norton, and M. A. Stewart, determined to organize and finish the task in hand, and Oxford University Press finally conceded it was time to replace Selby-Bigge. With the appearance of Tom Beauchamp's "critical edition" of the second Enquiry (hereafter identified as EPM), published on 4 August 1999, but bearing the date 1998, the long wait for a reliable text, bibliographic apparatus, and ancillary scholarship, worthy of Hume, must be declared over.If we grant that it is appropriate to introduce the new Clarendon Edition of the Works of David Hume with the piece he deemed "best," it must also be asked why he made this judgment. Philosophers have generally held that his Treatise of Human Nature (1739-40) is the most profound and searching statement of his epistemological and moral thought, but its seeming lack of immediate success with readers made Hume dissatisfied with it. Such were his negative feelings, coupled with irritation at attacks on him focused on this "juvenile" work, that he partially repudiated the Treatise at the end of his life. He wrote an "Advertisement" to this effect, and it was inserted as a preface to the authoritative and posthumous 1777 edition of his Essays and Treatises. Here he stated he was "sensible of the error of going to the press too early, and he cast the whole [of [End Page 280] the Treatise] anew in the following pieces [including EPM], where some negligences in his former reasoning and more in the expression, are, he hopes, corrected" (EPM Intro., xii-xiii, n.4).On the evidence of Beauchamp's editorial labors, it would appear that EPM is Hume's "best" work in this sense. It represents the most thorough recasting of source material provided by the Treatise. Its essential moral principles and psychological insights about the passions are retained, but "readability and elegance" are achieved by pruning, simplifying, and compacting the Treatise's complex teaching, while adding a wealth of illustrative material drawn from Hume's mature reflection on his subject and...
